What are the standard fields that you require the user to fill out in order to enter the contest or a sweepstakes?

Below are the standard fields included in the user registration.. Fields with an "*" are required.

• First Name
• Last Name
• Phone Number

• Address1
• Address2
• City
• State
• Zip/Postal Code
• Agree to rules

We also have an option of Newsletter opt-in. Adds an opt-in check box for newsletter subscription. Check newsletter by default
